- 1Visit pacjent.gov.pl and sign in to your Internet Patient Account (IKP), for example with a Trusted Profile.
- 2Open “Moje konto”, then “Twoja Podstawowa Opieka Zdrowotna (POZ)” and select a new primary care doctor declaration.
- 3Find our clinic using the name and address below. Choose a doctor and complete the declaration.
- 4Check your details, sign and submit the declaration. Check its acceptance status in IKP.
