ONLINE REGISTRATION

Using the portal of the Ministry of Health and the NFZ, you can complete an online declaration to register with a Primary Care Physician. By registering with the portal, we can also monitor issued e-prescriptions and their digital codes, e-sick leaves, as well as e-referrals on an ongoing basis.

Completing the selection declaration for a primary care physician and a community nurse is necessary to use clinic services under NFZ benefits. You may also complete and submit the declaration at our clinic's reception desk.

To register on the portal, please visit the website https://pacjent.gov.pl

After entering the website, log in to the Online Patient Account (IKP) by clicking the “zaloguj się” (log in) button in the upper right corner of the page.

 

To log in, you will need a Trusted Profile (Profil Zaufany). If you do not have this profile, the instructions for creating one are located under the “zaloguj się” button on the page below. To set up a profile, a bank account/e-ID is sufficient, or as a last resort, a visit to a government office.

Once logged into the IKP, you can fully utilize its features. To complete the primary care physician declaration, scroll down to the bottom of the page.

After scrolling down the page, go to “Moje konto” (My account), and then click the “zobacz” (view) button.

In the “moje konto” tab, go to the Primary Health Care (POZ) section, and then successfully complete the Physician declaration, followed by the Nurse declaration.

After clicking “wypełnij deklarację” (fill out declaration), you will be taken to a new tab where you must fill in the fields marked with a red asterisk. For certainty, it is better to update your contact details in the “moje konto” tab before starting the declaration.

To register with Primary Care Physician Mariola Karolak-Tomczuk, please complete the declaration as shown in the image below:

Next, click the “podpisz deklarację” (sign declaration) button.

After completing the Physician declaration, you should similarly complete the selection declaration for a community nurse.

Once signed, you can use the clinic's services under NFZ benefits. You can schedule an appointment online by visiting the following page:
