As part of healthcare in Poland, all patients covered by NFZ insurance have the option to choose their own primary care physician. This is an option available to every person, regardless of age. An additional right resulting from membership in the fund is also the ability to change your physician. However, there are specific conditions that must be met for the change to be free of charge. It is worth knowing them if we need to choose a new primary care physician.

When registering yourself or a child at a given primary healthcare (POZ) facility, each of us is required to complete an NFZ declaration, which enables the patient to choose free services such as:
- choice of a primary care physician
- choice of a nurse
- choice of a midwife
The necessary condition that must be met for the above primary healthcare services to be available to us free of charge is joining the NFZ fund, i.e., paying monthly contributions. The Act of July 1, 2020, also specifies specific rules for changing the selected primary care physician. Each beneficiary may choose their POZ physician no more than twice in a given calendar year. In the NFZ declaration submitted at the clinic, you must also indicate the reason for the change or provide your own reason. It is worth knowing that the choice of justification itself does not affect the decision of a given facility to assign a specific doctor. In the declaration, you must provide the first and last name of the doctor you want to be your primary care physician. The change we indicate takes place automatically after the declaration is submitted by the patient (or their guardian in the case of minors).

Every patient covered by NFZ insurance has the right to change their POZ physician multiple times using the aforementioned declaration. However, it is worth remembering that depending on the reason indicated in the declaration, a person may be exempt from paying an additional fee in the event of making a third change of doctor within a year. According to the act, such situations include:
- change of residence
- cessation of services by the doctor we selected
- reasons on the part of the healthcare provider (e.g., liquidation of the facility, the doctor leaving)
- the patient reaching the age of 18
If the reason lies with the beneficiary, i.e., the patient, choosing a third doctor within the same calendar year will involve a fee of 80 PLN.
